The purpose of the study is to create a registry (list) of participants who are interested in taking part in future research related to Alzheimer’s Disease and other memory disorders.

Participants will complete a survey to evaluate their interest in and eligibility to participate in future research, including questions regarding past medical history, psychiatric history (depression, anxiety), family history and its relationship to memory problems, and medications. The study includes completion of one questionnaire to enter the registry and an annual follow-up call to confirm interest for continued participation.
