For an initial diagnosis, send the specimen and a cover letter outlining the clinical history and the gross description of the specimen. For second opinions, please also include a copy of the surgical pathology report, even if incomplete, to document patient identity and correlate slides. Ship slides in secure holders and padded envelopes, or ship paraffin blocks. Call 310‑423-6623 for instructions if sending something other than slides or blocks.
Please also include front and back copies of the patient insurance card. Cases submitted without insurance information will be billed to the referring physician.
