How to Find Skilled Nursing Care That Fits Your Needs

We want to make sure you get the right care to help you heal after you leave Cedars-Sinai. Your care team will help you start planning for your discharge early in your hospital stay.

There are different care options available once you are ready to leave the hospital. Understanding these options will help you and your care team plan for the type of care that is best for you.

Most people plan to go home from the hospital. Based on your health and living situation, you may need to go to a care facility before you are well enough to go home. We will talk with you and your doctor to help you find the right place.

There are 4 types of care facilities, depending on the care you need after you leave the hospital. This website will help you learn more about each type of care facility.

Offers skilled nursing care 24 hours a day, 7 days a week. Most people stay at a skilled nursing facility for 1 to 2 weeks before going home.

Provides more specialized care than a skilled nursing or rehab facility.

Offers inpatient care and rehabilitation for patients with an acute illness, injury or disease or complex health problems.

May be needed after undergoing certain types of surgery, such as brain or spine surgery. Acute rehab also may be right for you if you have had an injury or a debilitating disease that keeps you from being able to walk, stand, bathe, dress or feed yourself.
