Application Payment

Thank you for your interest in joining the Medical or Allied Health Professional Staff at Cedars-Sinai. Please review the following instructions before submitting your application payment. 

New Applicants to the Medical Staff

You are required to pay a $250 non-refundable fee in order to be mailed an application packet. On the "Make Payment" page, enter the appropriate credentialing process to get started; select "Pre-Application" in the drop-down menu for fee type. When you return your application, the Application Processing fee of $500 will be due.

New Applicants to the Allied Health Professional Staff

Please choose your specialty from the drop-down menu on the payment page to display amount due.

Reappointment/Reappraisal

If you are paying dues as part of your Reappointment/Reappraisal Application, please choose your specialty from the drop-down menu on the payment page to display the amount due.
