Thanks to state‑of-the-art technology and specialized care teams, eligible patients can choose to receive care in the comfort of their home by enrolling in Cedars-Sinai @ Home.
If you and your doctor agree this program is right for you and you decide that you want to participate in the program, we’ll arrange to take you home—or to the house of a loved one, if that’s where you’ll be receiving care.
What to Expect
Our team will set up the home to get it ready for advanced care. Once that’s done, here’s what you can expect from Cedars‑Sinai @ Home while you’re enrolled in the program:
- 24/7 access to care
- Home visits and virtual appointments
- Remote monitoring of your vital signs
- A special command center led by doctors and other healthcare professionals who coordinate your care
- A technology kit, available to you while you’re enrolled in the program and used exclusively for your care, that makes it easy to connect with your care team and that helps them track how you’re doing
- Medical equipment such as a bed, shower chair, blood pressure monitor, pulse oximeter and more, also available to you while you’re enrolled in the program and used exclusively for your care
- Tests, including lab work
- Pharmacy/medication management, including IV therapies
- Interpreter services, if needed
- Personalized daily schedule continued
