All Cedars-Sinai Medical Group physicians and their phone numbers are listed in the Medical Network Services Provider Directory. You can review where a physician attended medical school and residency, or find out what languages other than English are spoken. To arrange an appointment, please call the physician's office directly, or contact our Customer and Member Services Department at (800) 700-6424 with any questions.
What to Bring to Your First Visit
Our physicians want to make your first visit with us a success and are interested in learning as much as possible about your current health status. Please arrive a few minutes early so you have time to fill out our health status forms, which help the physician learn about any areas requiring care. Filling out these forms will help your physician make the most of your time together.
If you are insured, the office staff will ask to see your insurance card.
You should bring to your first visit the bottles of all medications you are taking.
If you have not already arranged for your medical records to be transferred, please do so as soon as possible (see instructions below).
Transfer of Medical Records from Another Physician to Cedars-Sinai Medical Group
If you are a patient who needs to have copies of medical records from a physician sent to a Cedars-Sinai Medical Group physician, you can print out this form (Medical Record Request). The form is in a .pdf format, so you will need Adobe Acrobat to read and print it. Once you have printed it, filled it out and signed it, please take it to your physician's office or fax it to (310) 247-4619.
Transfer of Medical Records from Cedars-Sinai Medical Group to Another Physician
